1. At the time of membership and training course :-

1. 5  passport size colour photos.
2. Residence proof copies.
3. Qualification proof copies.
4. Application letter in written  for  in which course applying.
5. Membership/ Course fee.

   Note:- Book material cost will be charged extra  in all courses.

2. Application for certified Practioner Registration :-

1. 6 passport size colour photos.
2. Experience certificate copy.
3. Character certificate.
4. Details about qualified in which disease.
5.  Appreciation letter from 5 patients.
6. Residence  proof, ID copy.
7. Qualification proof copies.
8. Life membership fee  5100/-.

3. Application for opening WARDC’s centre/branch :-

1. Application for opening  centre/ branch.
2. Study Centre (incl. office, library, OPD, training & study rooms etc.)
3. Full rough map, photographs  of study centre.
4. NOC or rent agreement copy.
5. Oath letter by Branch Chief/ centre operator.
6. Centre registration fee  11000/- Or  admission fee of 50 students in advance.
7. Distt. Branch permission fee  51000/- Or 25 Life members fee in advance.

    Note :- 1. If any organization  want to  run WARDC’s centre, the organization Chief  will have to give 
                      a written application on letter-pad with registration copy.

               2.  In a distt., many study/training centre will be open.  But  Distt. Committee will be single
                    organized by Central Office. And the all  programmes will be run under the guidelines of
                   Central Head Office.

4.  Affiliation for organization :-
     If you run your own Ayurved/herbal or desi clinic/hospital/beauty parlour/arogya Kendra/
     Institute/Pharmacy/ study centre/ firm/company/ society/ trust etc. And want to adjoin with
     WARDC, so you may fulfill the below formalities:-

1. Organizational application letter.
2. Registration copy of your organization.
3. Printed prospectus/rules of your organization.
4. Details about organizational activities or specialties.
5. List of board members.
6. Oath letter by Organization’s controller.
7. Life Membership/affiliation  fee  5100/-.
